The entire film centers itself around the boy, Cole, and his new acquaintance with child psychologist, Malcolm. Bruce Willis' Malcolm does his best to try and help understand the boy, hoping he wouldn't fail his new patient as he had done with a patient in the past, while his marriage is seemingly on the rocks.
Though nothing seems to be amiss for the entirety of the movie, other than Cole's disturbing ability to see ghosts, audiences are in for the surprise of their life when it's revealed at the end that the determined Dr. Malcolm Crowe isn't alive. The plot twist was revered as a groundbreaking scene for years and stands up to current standards as one of the strongest moments in storytelling.
